Day Trip Volunteers (Most field trips)

NEED THESE FOUR DOCUMENTS:



1. PA State Police Clearance: https://epatch.pa.gov/home

2. Child Abuse Clearance: https://www.compass.dhs.pa.gov/CWIS (Must be done on a laptop or computer. Does not work on a mobile phone) This MUST be the PA Child Abuse History Certification. It CANNOT be the Department of Human Services fingerprint-based FBI Criminal History Background Check.

3. FBI Fingerprint Clearance: https://uenroll.identogo.com/ Use code 1KG6ZJ

*If you have lived in PA for the last 10 years, you can sign the Disclosure Statement instead of obtaining your FBI clearance.

4. Policy 916: Read, sign, and return only the signature page.

Overnight Trip Volunteers (HS band trip)

NEED THESE EIGHT DOCUMENTS:

4. Act 126 Mandated Reporter Certificate: * If you do not currently hold this certificate, you can access the University of Pittsburgh's 3 hour course at no charge here. https://www.reportabusepa.pitt.edu/

5. Act 24 Arrest and Conviction Report: Complete, sign, and return.

6. TB Test: done by your physician within the last 3 months. Fax results to 814-849-6842

7. Policy 916: Read, sign, and return signature page.

8. Completed Volunteer Form: Complete, sign, and return.
